    Sex Coaching Informed Consent

    INTRODUCTION

    We understand the importance of taking this first step and feel honored that you are trusting TBD Health Inc. (“TBD”) with your coaching journey. TBD believes that sexuality is an essential part of the human experience – and our goal is to support you in your personal goals for sexual wellness. Safe sex is: consensual, free of coercion, validating of all bodies, inclusive of all abilities, free of discrimination, and fundamental to overall health.

    The purpose of this informed consent form (this “Informed Consent”) is to: (1) obtain your permission to complete sex coaching session(s); (2) provide you with important information about these professional coaching services; and (3) outline our business policies. Please read it carefully and discuss any questions you have with your sex coach before beginning coaching.

    By booking an appointment, and attending a coaching session, you are consenting to this Informed Consent, and you acknowledge, understand, and voluntarily consent to the statements made in this form and to our performance of a sex coaching services.

    IF YOU HAVE AN EMERGENCY, IMMEDIATELY CALL 911. TBD’s coaching services are not appropriate for emergency care or urgent situations.

    SEX COACHING

    Sex coaching is a confidential process wherein a client works with a certified sex coach involved in addressing sexual concerns, sexual wellness, and sexual healing. The coach works to motivate clients by setting goals and providing feedback and accountability to clients. The outcome of sex coaching is often positive; however, the level of satisfaction for any individual is not always predictable. Your coach is available to support you throughout the process. We ask that you invest yourself in the process and commit to using the tools we talk about in our coaching sessions. When participating in group coaching, if applicable, all clients commit to the same confidentiality to support group cohesion.

    Please note that sex coaching is aimed at providing education tools, skills for improved individual and partnered sexual experiences, and these interventions are not counseling, psychotherapy or healthcare. If you desire more intensive sex therapy, please let us know. Your coach or another TBD representative can provide you with referrals in your state.

    COACH INFORMATION

    Your sex coach is Dr. Sophia Murphy, DBH, LPC, CPT, SXI. She is a licensed as a Professional Counselor by the Arizona Board of Behavioral Health, a certified Sexologist by the American College of Sexologists, and a licensed Personal Trainer by The National Academy of Sports Medicine. She holds a Masters degree in Marriage and Family Counseling from San Francisco State University and a Doctorate Degree in Behavioral Health from Arizona State University. She will draw from her training and experience in mental health, physical fitness, and sexual health to provide education and intervention in support of overall sexual wellness.

    CLIENT RIGHTS

    All clients will be treated equally without regard to gender, age, race/ethnicity, sexual orientation, religion, and/or disability. As a client, you will be treated with dignity and respect.

    PURPOSE, LIMITATIONS AND RISKS

    Participation in coaching can result in many benefits to you, including improving interpersonal sexual relationships and resolution of your specific concerns. The results you obtain from coaching will largely depend on your commitment to your growth process. Therefore, homework may be given to continue your growth process between sessions. Coaching requires your very active involvement, honesty, and openness in order to change your thoughts, feelings, and behavior. During coaching, talking about unpleasant events, feelings, or thoughts can result in considerable discomfort or experiences of anger, sadness, worry, fear, anxiety, depression, insomnia, etc. Your coach may challenge some of your perceptions or propose different ways of thinking about sex which can cause you to feel upset, angry, depressed, challenged, or disappointed. Attempting to resolve issues that brought you to coaching may result in changes that were not originally intended. Coaching may result in decisions about changing behaviors, relationships, etc. Sometimes a decision that is positive for one person is viewed negatively by another person in your personal life. Change will sometimes be swift and most often it will be gradual. There is no guarantee that coaching will yield positive or intended results. While your coach is also a counselor, coaching sessions are not counseling nor psychotherapy. A client who is interested in initiating a therapy relationship will be provided external referrals upon request.

    CONFIDENTIALITY

    All information disclosed within sessions and records pertaining to those sessions are confidential and may not be revealed to anyone without your written permission except where disclosure is required by state law and professional guidelines. These exceptional circumstances include: (1) reasonable suspicion of physical, emotional, and/or sexual abuse or neglect of a minor; information regarding a minor’s exposure to or participation in illegal activities with adults; (2) reasonable suspicion of physical, emotional, financial and/or sexual abuse or neglect of a dependent adult or elderly person; (3) reports of the intention to harm self and/or others; (4) cases where client records are court subpoenaed; (5) a provider may also contact the person whose name you have provided as your emergency contact; and (6) in partnered coaching, when the same coach sees partners individually, confidentiality and privilege do not apply unless otherwise agreed upon. Your coach will use their judgment when revealing such information.

    CONSULTATION

    Coaches may also seek consultation and or supervision from professionals inside or outside the practice regarding client care, best practices, education, and potential referrals. The providers at TBD protect each client’s identity, and therefore confidentiality is fully maintained.

    RECORDS AND YOUR RIGHT TO REVIEW THEM

    Professional coaching standards require that all coaches keep coaching records. Your records may include phone and email communication between you, coach, office staff, and (if a release is signed) with other individuals involved in coaching. Any messages that you leave on voicemails, text messages, and/or emails may be printed out or transcribed and entered into your records at any time to ensure proper documentation.

    You have the right to receive a summary of your records at any time, except in limited legal or emergency circumstances or when your coach assesses that releasing such information might be harmful to you in any way. Considering all of the above exclusions, if it is still appropriate, and upon your request, your coach will release information to any agency/person you specify.

    When more than one client is involved in coaching, your coach will release records only with signed authorizations from all the adults (or all those who legally can authorize such a release) involved in the coaching. For legal and documentation purposes, the coach must fill out a release of records, which will be reviewed and signed by the coach or a representative at TBD.

    PROTECTING THE PRIVACY OF YOUR INFORMATION

    Your privacy is important to TBD.     TECHNOLOGY

    You will be completing your coaching session virtually, which may involve the use of questionnaires, audio, video, or other electronic communications by your coach for the purpose of interacting with you, consulting with you, follow-up, and/or education. You will also have the option during each session to opt into it being recorded.

    Electronic systems used during your coaching encounter will incorporate network and software security protocols to protect the privacy and security of your information, and will include measures to safeguard your data to ensure its integrity against intentional or unintentional corruption.

    ACKNOWLEDGEMENT OF FINANCIAL POLICY

    Accepted forms of payment: PAYPAL & CREDIT CARD 1) I understand that payment is due in full at the time of the service being provided (if so applicable). 2) I understand that I am financially responsible for all services provided.


    By continuing to schedule a sex coaching appointment, and attending a session, I hereby, confirm that:
    1) I have read or have had read to me, all of the information in this Informed Consent document, and I understand what it says.

    2) I have had the opportunity to ask any questions I may have, and all of my questions have been answered to my satisfaction.

    3) I understand I have the right to refuse to sign this Informed Consent or to revoke my consent in writing at any time. I understand if I refuse to sign or revoke my consent TBD cannot provide any of the services described herein. I understand that this Informed Consent shall remain in effect until it is revoked by me.

    4) I acknowledge my consent as noted above, and I further recognize that this Informed Consent will supersede any other consent forms with a date prior to the date written below. My consent may be revoked at any time by written request to hello@tbd.health
